Personal information collected includes name, date of birth, age, gender, occupation, address, email address, phone number, current and past health, personal history, family history, medications, dietary intake, lifestyle information, pathology reports, payment information, internet usage information, and miscellaneous information collected through email discourse, text and social media. We collect your personal information through using cookies, when you subscribe to our mailing list, booking in consultations with us, booking discovery calls with us, correspondence via social media, correspondence via email, referrals from other health practitioners and modalities (with your consent). Personal information may also come from a guardian, a partner and other healthcare professionals such Gp’s and diagnostic imaging services. You can choose to disable cookies when accessing websites, but it may affect the website experience.
